Wallace, MD, MPH, Director Department of Public Health and Environment COLORADO DATE: October 8,2012 SUBJECT: A35-Weld County—Task Order Contract PO/Encumbrance NO. —PO FLA PPG1348329 Enclosed for Board review and approval is the A35-Weld County Task Order Contract- PO FLA PPG 1348329 between the Colorado Department of Public Health and Environment and the Weld County Board of Commissioners for the use and benefit of the Department of Public Health and Environment's Public Health Tobacco Program to provide tobacco cessation, reduce initiation of tobacco use, decrease tobacco prevalence among youth and decrease exposure to secondhand smoke. The task order contract is for funding in the amount of$255,208 for the period October 22, 2012 through June 30, 2013. "Me scope of work for this contract is to: 1) reduce the cessation success gap affecting low SES youth and adult smokers; 2)decrease tobacco prevalence and initiation among young adults, especially straight to work young adults; 3)prevent initiation among youth, but especially among high burden and low SES youth: and 4)educate the public and community stakeholders on evidenced-based strategies which have been shown to deter tobacco usage especially among youth. The project has eight objectives(outlined in the attached work plan) that will be performed by two Health Education Specialists(2 FTE, not existing but approved previously and sunset reviewed)and .25 FTE Office Technician (existing). Personnel expenses comprise 44% ($1 13,409)of the grant funds with the remaining funds being used for operating, contractual, travel, indirect and other costs. Tobacco use continues to pose a serious health threat to Weld County residents. Among the five greatest risk factors for mortality, it is the single most preventable cause of death and disease. Approximately 250 —300 county residents die every year from tobacco-related illnesses(i.e.,cancer of the lung and bronchus, chronic obstructive lung disease) and another approximately 5,000 —6,000 county residents may suffer from a serious tobacco-related illness. Nearly all tobacco use begins in childhood and adolescence.Nationally, about nine out of ten adults smokers who smoke daily report that they started smoking by the age of 18 years and nearly all first use of cigarettes occurs by 26 years of age. Most young smokers become adult smokers and one-half of adults smokers die prematurely from tobacco-related diseases. As Weld County's population continues to grow and its demographic makeup continues to change, reaching our more vulnerable priority populations(i.e., youth, young adults, etc.) with tobacco prevention interventions and cessation services is very important. I recommend your approval. 2012-2936 Enclosure DIP.AIl"IT[I INT Oh PUBLIC I IF..AVII I AND ENVIRONNIF:NT ROUTING NO.13 FLA 48329 APPROVED TASK ORDER CONTRACT-WAIVER#154 This Task Order Contract is issued pursuant to Master Contract made on 01/17/2012,with routing number 13 FAA 00051 5r AI Is <ON1R ACti OR State of Colorado for the use& benefit of the Board of County Commissioners of Weld Department of Public Health and County (a political subdivision of the state of Environment Colorado) PSD-Tobacco 915 101^ Street 4300 Cherry Creek Drive South Greeley, Colorado 80632-0758 Denver,Colorado 80246 for the use and benefit of the Weld County Department of Public I lealth and Environment 1555 17th Avenue Greeley,Colorado 80631 I \1k ORDI-.R M-ADI Ill II(: CON I R VCtfR[Nil IV I)PI(. 10/05/2012 Colorado Political Subdivision Pt)/SC I;NCImIte vNi I NI.NII1I PC) FLA PPG1348329 [F RN[ 01L1.INC SI MAUNA',iA[I-N IS RI(LIVED "Ibis Task Order shall be effective upon Monthly approval by the State Controller, or designee, or on 10/22/2012,whichever is later.
