RE: APPROVE THREE PURCHASE OF SERVICES AGREEMENTS FOR OPERATIONAL
ACTIVITIES WITH VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with three Purchase of Services Agreements for
Operational Activities between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Human Services, Area
Agency on Agency, and the various Senior Centers, as follows, commencing November 1, 2012,
and ending June 30, 2013, with further terms and conditions being as stated in said agreements,
and
Hill N Park Senior Citizen Program
Lochbuie After Glows Senior Citizens Center
Pierce Senior Citizens, Inc.
WHEREAS,after review, the Board deems it advisable to approve said agreements,copies
of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the three Purchase of Services Agreements for Operational Activities
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services,Area Agency on
Agency, and the various Senior Centers as listed above be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said agreements.

RE: Weld County Department of Human Services, Area
Agency on Aging's Operational Grant
Enclosed for Board approval are the FY13 Operational Grants between the Department and
Various Providers. These were reviewed at the Board's May 10, 2012, Work Session.
Hill N Park, Lochbuie AfterGlows, and Pierce Senior Center are on a very limited budget
and need assistance with basic operational expenses such as utilities, insurance, office
supplies, postage, etc. All three sites provide free space and volunteers for the Area Agency
on Aging's (AAA) Senior Nutrition meal sites. Without these senior centers, the AAA
would be unable to provide the meal program for seniors residing in these communities.
The effective date of these agreements is November 1, 2012 through June 30. 2013. The
amount of funding for each is as follows:
• Hill and Park Senior Center $500
• Lochbuie AfterGlows Senior Center $500
• Pierce Senior Center$500

Purchase of Services Agreement
THIS AGREEMENT, made and entered into this P'day of November, 2012 by and between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Division of
Human Services' Area Agency on Aging, hereinafter referred to as "AAA" and the Hill N Park Senior Citizen Program
hereinafter referred to as "Hill N Park."
WITNESSETH:
WHEREAS, "AAA"develops and administers the Area Plan for a comprehensive and coordinated system of services, and
WHEREAS, "AAA"considers small rural senior centers a vital link in the county's system of services to older adults, and
WHEREAS, "Hill N Park" is a senior center providing recreational, nutritional, and educational opportunities for senior
citizens;
NOW THEREFORE, it is mutually agreed:
1. The"AAA" hereby agrees to award grant monies(Older Coloradoan Fund) in the sum of$500 to "Hill N Park"
2. "Hill N Park" agrees to expend the grant monies to pay for operating expenses for the center to include: electricity,
gas, telephone, insurance, office supplies, postage, and other minor operating expenses.
3. "Hill N Park"agrees to maintain documentation of the above expenditures and to seek prior approval from the AAA
if the expense source is not listed in the statement above.
4. "Hill N Park" understands that the terms of the agreement are valid through June 30, 2013
IN WITNESS WHEREOF, the parties hereunto have caused this Agreement to be duly executed as of the day and year
first hereinabove set forth.
