Date: October 17, 2012
Please find attached an addendum to our contract with the Decade Software Company. I
recommend that you authorize the addendum.
The addendum is necessary to update the our contract with Decade to reflect the actual number
of users currently using the software. Decade is the software used by the Health Department
for management of restaurants, childcare, body art, etc. facilities. The addendum will authorize
up to 10 users (which is the current number of users in the Department). The current contract
authorizes only 5 users. The current annual cost is $15,300. It will increase to $18,900.

Addendum
This is an Addendum(hereinafter"Addendum")to the License and Support Agreement effective
February 20,2008(hereinafter"Agreement")between Decade Software Company,LLC with principal
place of business at 1 195 West Shaw Avenue, Fresno,California 93711 ("Decade"),and Weld County
Health Department("Client")with principal place of business at 1555 North 17th Avenue,Greeley, CO
80631.
This Addendum shall be incorporated in its entirety into the Agreement. To the extent any provision
contained in this Addendum conflicts with any provisions contained in the Agreement,the terms in this
Addendum shall control.
1 . Number of Inspectors
This Addendum becomes effective September 1, 2012 and increases the number of Full Time Equivalent
(FTE) field inspection staff from five(5)to ten(10),as follows:
