RE: APPROVE TWO (2) AMENDMENTS TO CHILD PROTECTION AGREEMENTS FOR
SERVICES WITH VARIOUS PROVIDERS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with two (2) amendments to Child Protection
Agreements for Services between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Human Services, and the
various providers listed in Exhibit"A", a copy of which is attached hereto and incorporated herein by
reference, commencing October 1, 2012, and ending May 31, 2013, with further terms and
conditions being as stated in said amendments, and
WHEREAS, after review, the Board deems it advisable to approve said amendments, copies
of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado that two (2) amendments to Child Protection Agreements for Services between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Human Services, and the providers listed in Exhibit "A", a
copy of which is attached hereto and incorporated herein by reference be, and hereby are,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendments.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 3rd day of October, A.D., 2012

SUBJECT: Amendments to Child Protection Agreements for Services
between the Weld County Department of Human Services and
Various Providers
Enclosed for Board approval are Amendments to Child Protection Agreements for Services between the
Department and Various Providers. These Agreement Amendments were reviewed under the Board's
Pass-Around Memorandum dated September 13, 2012, and approved for placement on the Board's
Agenda.
The major provisions of these amended Agreements are as follows:

AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES AND
REFLECTIONS FOR YOUTH,INC.
This Amendment,made and entered into the day of September 20I 2,by and between the
Board of County Commissioners,on behalf of the Weld County Department of Human Services,
hereinafter referred to as"Department,"and Reflections for Youth, Inc.,whose address is P.O.Box 1860,
Berthoud,CO 80513,hereinafter referred to as the"Contractor."
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement for Home Based Intensive
Services(PY-12-13-CORE-0206)in order to add an additional level of service under this agreement;and
WHEREAS, Human Services has budgeted,appropriated,and otherwise made available;and
WHEREAS,required approval,clearance,and coordination have been accomplished from and
with appropriate agencies.
NOW THEREFORE,in consideration of the premises,the parties hereto covenant and agree as
follow:
I. Exhibit A,Scope of Services,Item 2b, is hereby revised to include the following:
Mentoring Level=5 hours/week.
2. Exhibit B,Item 2,Fees for Service,is hereby revised to include the following:
$165.00/Week(5 Hours/Week without Therapy)
This Amendment,together with the original Child Protection Agreement for Services,constitutes
the entire understanding among the parties with respect to the subject matter hereof,and may not be
changed or modified except as stated in the Child Protection Agreement for Services.

TURNING POINT CENTER FOR YOUTH AND FAMILY DEVELOPMENT, INC.
This Amendment,made and entered into the day of September 2012,by and between
the Board of County Commissioners,on behalf of the Weld County Department of Human
Services,hereinafter referred to as"Department,"and Turning Point Center for Youth and Family
Development, Inc.,whose address is 1644 South College Avenue, Fort Collins, CO 80525,
hereinafter referred to as the"Contractor."

WHEREAS,the parties desire to modify their current Agreement for Day Treatment.)
Services(PY-12-13-CORE-0175) in order to decrease the rate of Transitions Services under this
agreement;and

WHEREAS,Human Services has budgeted,appropriated,and otherwise made availaS;

WHEREAS,required approval, clearance,and coordination have been accomplished
from and with appropriate agencies.
NOW THEREFORE, in consideration of the premises,the parties hereto covenant and
agree as follow:
1. Exhibit A,Scope of Services, Item 2, is hereby revised as follows:
CONTRACTOR will provide day treatment services, as proposed,at 913 11th
Avenue, Greeley, CO 80631, 8:30 a.m.to 4:30 p.m., Monday through Friday, or
as agreed to by the Department for purposes of transition to public school.
2. Exhibit B,Item 2, Fees for Service, is hereby revised as follows:
$55.00/Day(Day Treatment-Transitional Services. Only used for clients
transitioning to public school.)
This Amendment, together with the original Child Protection Agreement for Services,
constitutes the entire understanding among the parties with respect to the subject matter hereof,
and may not be changed or modified except as stated in the Child Protection Agreement for
Services.
