RE: APPROVE APPOINTMENT OF PRIVACY OFFICERS UNDER HEALTH INSURANCE
PORTABILITY AND ACCOUNTABILITY ACT (HIPAA) AND SECTION 3-15 OF THE
WELD COUNTY CODE
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, by Ordinance #2012-10, the Board of County Commissioners approved the
Health Insurance Portability and Accountability Act (HIPAA) Policies and Procedures for Weld
County, and
WHEREAS, Section 3-15-30 of the Weld County Code appoints the Director of Finance
and Administration as the overall Weld County Privacy Officer, and
WHEREAS, the Board has previously appointed individuals to be departmental HIPAA
Privacy Officers, some of which are no longer Weld County employees, and some of which are
not employed in any of the covered departments, as defined in Section 3-15-20 of the Weld
County Code, and
WHEREAS, the Board deems it advisable to revoke all prior appointments of HIPAA
privacy officers, and to appoint privacy officers in each of the covered departments, and
WHEREAS, the Board deems it advisable to make the following appointments, with such
appointments superseding all prior appointments as HIPAA Privacy Officers.
NOW, THEREFORE BE IT RESOLVED by the Board of County Commissioners that
Monica Mika, as the Director of Finance and Administration, be, and hereby is, appointed as the
HIPAA Privacy Officer for Weld County.
BE IT FURTHER RESOLVED, by the Board that Mark Wallace, Director of the
Department of Public Health and Environment, be, and hereby is, appointed as the HIPAA
Privacy Officer for the Weld County Department of Public Health and Environment.
BE IT FURTHER RESOLVED, by the Board that Patricia Russell, Director of the
Department of Human Resources, be, and hereby is, appointed as the HIPAA Privacy Officer
for the Weld County Department of Human Resources.
BE IT FURTHER RESOLVED, by the Board that Nancy Kroll, Director of Inmate
Services for the Weld County Sheriff's Office, be, and hereby is, appointed as the HIPAA
Privacy Officer for the Weld County Sheriff's Office.
BE IT FURTHER RESOLVED, by the Board that Eva Jewell, Division Head for the Area
Agency on Aging, be, and hereby is, appointed as the HIPAA Privacy Officer for the Weld
County Department of Human Services, Area Agency on Aging.

BE IT FURTHER RESOLVED, by the Board that Barbara Connolly, Weld County
Controller, be, and hereby is, appointed as the HIPAA Privacy Officer for the Weld County
Department of Accounting.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 19th day of December, A.D., 2012.
