Pursuant to the provisions in our Agreement, Savio House is terminating their Child Protection
Agreement for Services with the Department due to unexpected changes within their office. This
Child Protection Agreement is Contract Number PY-11-12-HS-0053 and a Document Number of
2011-2317.

Good morning,Judy.
Please note that Savio House is terminating their Home Study/Relinquishment Counseling contract with us due to
unfortunate circumstances related to personnel. They are unable to comply with the SAFE Vendor List requirement and
do not anticipate being able to do so in the future. To that end, they have determined that it is best if they terminate
the contract at this time. Please note they also do not plan to reapply for services for Program Year 2012-13.
This is a Child Welfare Administration funded contract, not Core (please see attached). Savio has not been able to
accept any referrals this year due to the aforementioned personnel circumstances so there is no outstanding services or
billing.
Please let me know if you have additional questions or require anything further from me regarding the termination of
this contract.

Hi Tobi
I wanted to follow up with you after we spoke last week about Savio fulfilling the Home Studies portion of our
current contract. After further internal discussion and additional unexpected changes within the CPA, we have
determined we cannot proceed with getting on the State vendor list which will preclude us from taking
referrals for this program.
I wanted to acknowledge how appreciative Savio is for being able to provide such a variety of services to
families in Weld County and we in no way want this to reflect poorly on us as a provider. It was absolutely our
intention when we wrote the proposal last year to fulfill the requirements.
I wanted to make sure we officially notified you of this decision. Please advise us on what, if anything, needs
to be done from this point forward.
Thank you for the opportunity to serve Weld County families we are honored to continue with the other array
of services offered.
