The Department of Planning Services has determined that this is not a major change to the approved USR and Minor Amendment for the Class II Disposal facility under case numbers USR-1708 and MUSR12-0003. No additional planning permits or approvals will be required as long as the washout facility is operated under the following parameters outlined in your letter: • No additional structures will be added to the facility. • No increase in traffic entering or exiting the site will be generated. • Only the inside of the tanks will be washed out with recycled water. • Wash out will take place over the existing concrete pad containment area. • Wastewater from tank wash operations will be collected via pipe and/or the existing pad trench and returned to the facilities waste collection system for disposal. Any future changes to the washout facility component or any other component of the operations would need to be reviewed by the Department of Planning Services and may require additional permitting through the County. In addition to our disposal operations, we also recycle a portion of the water transported to our facility for reuse in oilfield operations. We would like to request a minor amendment to our USR expanding our services to include washing -out frac tanks and water trucks with our recycled water. Please note: No additional structures will be added to our facility; in fact, we will be utilizing our existing structures for the requested operations stated above. All sediment from tanks and trucks, along with any solids, will be placed into a dumpster with decanting capabilities. This will recover any water which can be again recycled. All dry solids will be transported to the nearest landfill for disposal and burial. With this expansion of washout operations incoming traffic will not be increased coming or going from our facility. Public Health & Environment DATE: June 10, 2013 SUBJECT: Lone Star, USR-1708, 1s` Semi -Annual Inspection 2013 The Department conducted a routine inspection on June 7, 2013 and found that the facility has temporality ceased injection due to having injected into the wrong formation. The well was being worked on at the time of the Department's inspection and the facility was still accepting limited loads for processing through the recycling plant based on demand for recycled wastewater. According to Select Energy Services, they expected to have injection disposal back in operation in two weeks. If they are saying they are injecting into the wrong formation, they are probably referring to the fact that the formation they picked won't take much water which makes it wrong for them. Denise M.
