SUBJECT: Emergency Flood Repairs Services Contract for
Bridge 9.5/24.5A Over the St. Vrain River - TLM Constructors
This memo shall serve as follow-up documentation for the emergency response and contract
award for TLM Constructors on the referenced bridge repairs.
During the course of the recent flooding between 9/11-9/20, the St Vrain River overtopped
the banks. Along with many other bridges, severe scouring occurred at Weld County Bridge
9.5/24.5. (This bridge parallels 1-25 and serves as the 1-25 Frontage Rd.) County crews
closed the bridge during the flooding until an inspection could be performed to evaluate the
bridge condition. State bridge inspectors performed their inspection on 9/16 and determined
that a large scour hole had formed under the NE approach slab (See photo) which was
unnoticeable from above by drivers. Due to the compromised structural integrity of the slab
and possible immediate failure,
they required this bridge to
remain closed until repairs could
be made.
Public Works was asked by the
Board of County Commissioners
to expedite a solution and
contract the work. Timeliness
was critical as this was the only
bridge crossing over the St Vrain
(besides 1-25) in several miles
either upstream or downstream
that did not receive extremely
large amounts of approach
damage. Mostly due to the fact
that 1-25 helped buffer it from
breaching the road approaches
to the bridge. This connection across the St Vrain would help reduce the traffic burden on
1-25 as well as help facilitate delivery of materials to the other damaged sites.
Weld County's standard procurement process would have resulted in delaying the repair
work for several weeks. Safety of the traffic was paramount and the slab could give way at
any time. Public Works contacted a local reputable vendor that has worked on bridges in the
County before and requested they develop an emergency scope of work to be submitted for
review and approval. Public Works reviewed the scope which only consisted of the minimal
amount of work to reopen the bridge. The contract was executed by TLM and Weld County
on Sept. 18 for the emergency repairs, in the amount of $48,053.00. They were able to
complete the work in 2 days and open the roadway to traffic.

RESOLUTION
RE: APPROVE AGREEMENT FOR COUNTY ROAD 9.5/24.5A OVER ST. VRAIN
EMERGENCY REPAIRS AND AUTHORIZE CHAIR TO SIGN - TLM CONSTRUCTORS,
INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Agreement for County Road 9.5/24.5A
Over St. Vrain Emergency Repairs between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Department of
Public Works, and TLM Constructors, Inc., commencing September 18, 2013, and ending within
ten (10) working days, unless the period for completion is extended otherwise by the agreement
documents, with further terms and conditions being as stated in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Agreement for County Road 9.5/24.5A Over St. Vrain Emergency
Repairs between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Public Works, and TLM
Constructors, Inc., be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said agreement.

AGREEMENT
WCB 9.5/24.5A Over St Vrain Emergency Repairs
THIS AGREEMENT, made this 18th day of September , 2013, by and between Weld County, Colorado,
hereinafter called "Owner" and
TLM Constructors Inc. doing business as (an individual, ) or (a partnership,) or (a corporation) hereinafter
called "Contractor".
WITNESSETH: That for and in consideration of the payments and agreements hereinafter mentioned:
1. The Contractor will commence and complete the following: WCB 9.5/24.5A Emergency Repairs.
2. The Contractor will furnish all of the material, supplies, tools, equipment, labor and other services
necessary for the construction and completion of the Project described herein.
3. The Contractor will commence the work required by the Contract Documents within ten (5)
calendar days after the Notice to Proceed and will complete the WCB 9.5/24.5A Emergency
Repairs within 10 WORKING DAYS unless the period for completion is extended otherwise by
the Contract Documents.
4. The Contractor agrees to perform all of the Work described in the Contract Documents and
comply with the terms therein for the sum of $48.053.00. (See attached Scope of Work).
5. The Owner will pay to the Contractor in the manner and at such times as set forth by the Contract
Documents, or otherwise agreed upon in writing.
6. This Agreement shall be binding upon all parties hereto and their respective heirs, executors,
administrators, successors, and assigns.
7. Contract Appropriations: The Owner hereby states and affirms that the amount of money
appropriated for this Contract is equal to or in excess of the Contract amount. No change order to
this Contract requiring additional compensable work to be performed, which work causes the
aggregate amount payable under the Contract to exceed the amount appropriated for the original
Contract shall be issued by the Owner unless the Owner assures the Contractor, in writing, that
lawful appropriations to cover the costs of the additional work has been made or unless such work
is covered under a remedy -granting provision contained in the Contract.
8. The Contractor understands and agrees that the work shall be in compliance with the attached
Davis Bacon Wage Rates.
9. Contractors work shall be in compliance with the latest CDOT Standard Specifications for Road
and Bridge Construction and the latest CDOT M & Standards.

WCB 9.5/24.5A Over St Vrain
Scope of Work
After your request to find a faster remedy I met with Les Matson at the structure at CR 9.5 and we
discussed the work you would like to complete.
What we came up with was to install approximately 24 LF of Sheet Pile along the wing wall, this will be
difficult because we anticipate that riprap is buried in the location that we will be installing the sheet
pile and it will have to be removed by hand. We anticipate that we will not be able to drive to the depth
needed to hold back the flow fill so we are anticipating having to install a steel whaler with cable tie
backs to contain the weight until the flow fill has set.
On the front of the abutment we will install ply form that will be stiffened using vertical steel whalers
that will be bolted to the abutment face and used in conjunction with horizontal whalers, we will not be
able to use "kickers" but feel this method will work to contain the flow fill.
Once forming is complete we will cut 4 holes in the approach slab that are 18 Inch square using a
diamond chain saw, this size of hole will provide us with better observation windows during flow fill
installation and access for concrete vibrators to consolidate the material as best we can. Once flow fill
installation is complete the hole will be patched using Quikrete DOT mix extended. This material will
obtain 2500 PSI in about 1.5 hours and 6000 PSI in 24 hrs.
Because of the weight of the flow fill the installation process will be slow. We will need to install several
2-3 foot thick lifts and let the material set before another lift is placed or we may push the forms and
sheet pile into the river.
Our estimated price for mobilization is $9,303.00. This includes mobilization costs for initial
construction and remobilization for removal of the sheet pile
Temp Sheet Pile Install and Extract $22,800.00. This includes installation and extraction of the sheet pile
as well as the tie backs as mentioned above.
Flow Fill Installation / Concrete Patching $15,950.00. This includes abutment forming, flow fill
installation, cutting holes and patching holes in the existing approach slab.
Total Estimated Cost for this work is $48,053.00
Les Matson is available tomorrow and I am not sure beyond that, as you can imagine he is getting busy
with all of the flood restoration work. If you would like to proceed I will make a crew available to get the

WAGE DETERMINATION APPEALS PROCESS
1.) Has there been an initial decision in the matter? This can
be:
* an existing published wage determination
* a survey underlying a wage determination
* a Wage and Hour Division letter setting forth a position on a wage determination matter
* a conformance (additional classification and rate) ruling
On survey related matters, initial contact, including requests for summaries of surveys, should be with the Wage and Hour
Regional Office for the area in which the survey was conducted because those Regional Offices have responsibility for the
Davis -Bacon survey program.
If the response from this initial contact is not satisfactory, then the process described in
2.) and 3.) should be followed.
With regard to any other matter not yet ripe for the formal process described here, initial contact should be with the Branch of
