WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with two (2) Amendments to Child Protection Agreements for Services (Day Treatment and Home Based Intensive Services) between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, and Reflections For Youth, Inc., commencing June 1, 2013, and ending May 31, 2014, with further terms and conditions being as stated in said amendments, and WHEREAS, after review, the Board deems it advisable to approve said amendments, copies of which are attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado that two (2) amendments to Child Protection Agreements for Services (Day Treatment and Home Based Intensive Services) between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, and Reflections For Youth, Inc., be, and hereby are, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said amendments. Griego, Director, Human S RE: Amendment to the Child Protectionigreement for Services between the Weld County Department of Human Services and Reflections for Youth Enclosed for Board approval is an Amendment to the Child Protection Agreement for Services between the Department and Reflections for Youth. This Agreement was reviewed under the Board's Pass -Around Memorandum dated December 5, 2013, and approved for placement on the Board's Agenda. There are two (2) Agreements being amended that include the Home Based Intensive Services and Day Treatment Services funded through Core. The agreement reflects administration changes at RFY, and the impact on services which were agreed to by both parties. There are no changes in rate or available service levels. The original Agreements were approved by the Board on September 23, 2013, for the period of June 1, 2013 through May 31, 2014. If you have any questions, give me a call at extension 6510. 2013-3512 a AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES BETWEEN THE WELD COUNTY DEPARTMENT OF HUMAN SERVICES AND REFLECTIONS FOR YOUTH, INC. This Amendment, made and entered into the day of November 2013, by and between the Board of County Commissioners, on behalf of the Weld County Department of Human Services, hereinafter referred to as "Department," and Reflections for Youth, Inc., whose address is P.O. Box 1860, Berthoud, CO 80513, hereinafter referred to as the "Contractor." WITNESSETH WHEREAS, the parties desire to modify their current Agreement for Day Treatment Services (PY-13-14-CORE-0112) in order to include additional parameters agreed to by both parties under this agreement; and WHEREAS, Human Services has budgeted, appropriated, and otherwise made available; and WHEREAS, required approval, clearance, and coordination have been accomplished from and with appropriate agencies. follow: NOW THEREFORE, in consideration of the premises, the parties hereto covenant and agree as 1. Exhibit A, Scope of Services, is hereby revised to include the following: 17. Clients receiving enhanced services will be allowed two (2) "no shows" or cancellations only. Once two (2) "no shows" or cancellations occur, CONTRACTOR will set an emergency staffing inclusive of all treatment team members. The staffing will assess whether or not services will continue and if the family is willing to engage. If determined that services will continue, referred client and/or family will be given one (I) additional opportunity to comply with services. If client and/or family "no shows" or cancels, the CONTRACTOR will discharge the client non -compliant from services. 18. CONTRACTOR will continue to provide enhanced services to clients detained or hospitalized at the location of detainment or hospitalization. CONTRACTOR will immediately address facility issues with the Department to ensure continuity of services. 19. CONTRACTOR will notify the referring caseworker and the Child Welfare Contract and Service Coordinator immediately if a client runs. The Department will authorize a three (3) day service hold, effective from the date the client runs. Clients will be staffed on a case -by -case basis to determine if services will be held open longer than three (3) days. This Amendment, together with the original Child Protection Agreement for Services, constitutes the entire understanding among the parties with respect to the subject matter hereof, and may not be changed or modified except as stated in the Child Protection Agreement for Services. Thereafter, staffings will occur twice per month so long as the client requires the higher level of intervention. CONTRACTOR will ensure staffings are inclusive of all treatment team members. 20. CONTRACTOR will notify the referring caseworker and the Child Welfare Contract and Service Coordinator immediately if a client runs. The Department will authorize a three (3) day service hold, effective from the date the client runs.
