Subject: Transportation Agreement with the Weld County Community Center
T6undation
Enclosed for Board approval is an Amendment to the Agreement with
the Weld County Community Center Foundation, Inc. , to provide trans-
portation services to their clients.
The purpose of this Amendment is to extend the length and cost of
the original contract first written on July 1 , 1977. This Agree-
ment shall be for a period beginning July 1 , 1979, and ending June
30, 1980. The Weld County Community Center Foundation will allo-
cate the amount of $15,000 to the Division of Human Resources to
transport their clients.

CONTRACT FOR PURCHASE OF SERVICES
AMENDMENT
THIS AMENDMENT, made and entered into this 1st day of July , 1979,
between the Weld County Community Center Foundation, Inc. , a non-
profit community incorporated board or center, duly incorporated
and existirg under corporate laws of the State of Colorado, here-
inafter called the Board, and the Weld County Division of Human
Resources , hereinafter called the Vendor.
WITNESSETH
WHEREAS, it is the desire of the Board and the Vendor to ex-
tend the length and cost of the original contract for purchase of
services entered into on July 1 , 1977; and
WHEREAS, it is also the desire of the Board and the Vendor to
extend the orginal contract under the same terms and conditions.
NOW THEREFORE, ALL CONDITIONS THAT FOLLOW ARE MUTUALLY AGREED
UPON:
1 . The term of this Agreement shall be from July 1 , 1979, to
June 30, 1980, unless cancelled or terminated as provided in (3)a
of the original contract for purchase of services entered into on
July 1 , 1977.
2. The Board promises to pay and hereby allocates to the Vendor
an amount not to exceed $15,000 in accordance with the method of pay-
ment as provided in the original contract for purchase of services
entered into on July 1 , 1977.
3. The parties hereto agree to extend the length and cost of
the Agreement under the same Terms and Conditions as first written
on July 1 , 1977.
IN WITNESS WHEREOF, the parties have executed this Amendment
on the day just above written.
