Please be advised that on June 18, 2013, L. G. Everist, Incorporated, whose address and telephone number are 7321 E.
88th Ave., Ste 200 , Henderson, CO 80640; (303) 286-2247, submitted a request for full reclamation responsibility
release for the Lupton Meadows Reservoir, Permit No. M-2002-104. The construction materials extraction operation
is located at or near Section 30, Township 2N, Range 66W, 06th Prime Meridian, in Weld County. The Lupton
Meadows permit is not reclaimed, but is now entirely incorporated into the amended Ft Lupton Sand and Gravel
Permit (M-1999-120) as shown by the attached maps.
Written comments or objections to the release may be submitted to, and additional information obtained from, the
Division of Reclamation, Mining and Safety, 1313 Sherman Street, Room 215, Denver, Colorado 80203, (303) 866-
3567. Comments or objections should be submitted within 15 days. The Division is required to wait 30 days after the
release decision is made to allow for appeals.

Re: Ft. Lupton Sand and Gravel Mine, Permit M1999-120, Amendment Approval, Revision AM -2
Dear Mr. Fields:
On October 25, 2012 the Division of Reclamation, Mining and Safety approved the Amendment application
submitted to the Division and amended on July 24, 2012, addressing the following:
Change in mining plan, addition of permitted/affected acreage, incorporate adjacent Lupton Meadows
permit.
The terms of the Amendment AM02 approved by the Division are hereby incorporated into Permit M1999-
120. All other conditions and requirements of Permit M 1999-120 remain in full force and effect.
The estimated liability amount of $2,002,400.00 exceeds the $997,872.00 performance bond currently held.
If you have not already done so, please submit additional bond in the amount of $1,004,528.00. The revision
will not be final until the bond is approved by the Division.
