USR Questionnaire
1. The property will be used for our personal living along with us parking 3 semi -tractor
trailers, with a possible 5 total for future growth. We haul crude oil. We will use a
shed (12X 20) to store miscellaneous spare parts i.e. tires. The trucks will only be
parked on property when not working. We run the trucks with drivers night and day.
Some of the time the drivers take trucks home depending on their loads for the day
or if we need to take trucks to Rush Peterbilt for any maintenance or repair work.
2. We are consistent with Weld County code, Chapter 22 because in section 22-2-20 .G.
Policy A.7.2 -Conversion of agricultural land to non -urban residential, commercial and
industrial uses should be accompanied when a subject site is in an area that can
support such development, and should attempt to be compatible with the region.
3. The Zoning —Article II, Division 4 of Weld county Code A. 2. We are oil and gas support
service.
4. The surrounding land use around us is farming and horse families. We are parked far
enough away not to disturb animals or farming property. We keep trucks and trailers
maintained and have annual inspections. Per DOT and public safety.

Traffic Narrative:
We have 3 Peterbilt semi -tractor trailers that we use daily. The trucks each weigh
approximately 30000 lbs. We use approximately a quarter mile of easement road that gives us
access to and from our property to County road 55. Monday through Saturday two of the trucks
leave heading east in morning, once these 2 trucks have left the drivers take them home at end
of day. We have 1 truck parked at house during day till midafternoon when it leaves for
approximately 8-10 hours before returning. On Sunday we have all trucks parked here during
day, till midafternoon then 1 truck leaves for work.
Waste handling
We use North Colorado disposal for our trash removal. They come out once a week.
Drivers have access to trash cans on property and they will have access to a portable toilet. We
do have tires on property but they will be used as retreads and then the old ones will be
disposed of at tire shop. We don't do our own maintenance so we don't store used oil or
antifreezes on property.
Dust abatement
We address this with slow speeds from trucks of 5 mph. We will add water to easement
road as needed to help control dust.
Storm water Drainage
We have lived on property for 18 months have never had issues with water draining. We
have had very little rain. We have never had drainage problems on property.

EASEMENT AND RIGHT OF WAY
THIS EASEMENT, granted this 21° day of August, 2003, by Roger C. Culbreath
and Nancy C. Cuibreath, Grantors, to owners of record of lots A and B (Grantees) of
Weld County Colorado Recorded Exemption No. 0801-21-4 RE -3610.
WITNESSETH:
Grantor hereby grants to Grantees, their successors and assigns, an easement and
right of way for the purpose of ingress and egress, in, through, over, under and across
Grantor's property situated in Weld County, Colorado described as Lot 2 of Recorded
Exemption No. 0801 -21 -3 -RE -105. Said easement and right of way being thirty (30) feet
on each side of a center line, the course of which is described as follows: Beginning at
the east quarter corner of Section 21, Twp. 6 North, Range 64 West of the 6th P.M., Weld
County, Colorado: thence South 30 feet to the true point of beginning: thence South
89°53"22" for a distance of 1744.97 feet for lot A of RE -3610 and along the same line
and from the true point of beginning a distance of 2,756.03 feet for Lot B of RE -3610.
Grantees shall share proportionately in maintenance, construction, or
refurbishment of access roads and placement or repair of utilities within the easement that
serve all lots of Recorded Exemptions RE -105, Lot 2 and RE -3610, Lots A and B.
Grantor warrants to Grantee that Grantor (a) has good and sufficient right and title
in and to the Property and full power to grant this easement and right-of-way, and (b) will
defend Grantee's quiet and peaceful possession of the Property and easement and right-
of-way against all persons who may lawfully claim title to the Property.
"Grantee" shall include the plural and the feminine. This Easement and Right of
Way shall be binding upon, and shall inure to the benefit of the heirs, personal
representatives, successors and assigns of the Grantor and Grantee.
