RE: APPROVE FIRST AMENDMENT TO AGREEMENT FOR INMATE HEALTH SERVICES
AND AUTHORIZE CHAIR TO SIGN - CORRECT CARE SOLUTIONS, LLC
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a First Amendment to Agreement for
Inmate Health Services between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Sheriff's Office, and Correct Care
Solutions, LLC, commencing January 1, 2014, and ending December 31, 2015, with further terms
and conditions being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the First Amendment to Agreement for Inmate Health Services between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the on behalf of the Sheriff's Office, and Correct Care Solutions, LLC,
be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 30th day of December, A.D., 2013.

FIRST AMENDMENT TO AGREEMENT FOR INMATE HEALTH SERVICES
This First Amendment (the "Amendment") to the Agreement for Inmate Health Services
by and between the County of Weld, Colorado ("Weld County") and Correct Care Solutions, LLC
("CCS") of original date November 30, 2012 (the "Agreement") is effective as of January 1, 2014.
WHEREAS, the Agreement may be amended pursuant to its terms as set forth in Article
XI (A) by written consent of the parties; and
WHEREAS, Weld County consents to an increase in compensation paid to CCS pursuant
to Article VI (A)(2) of the Agreement to accommodate the rising cost of healthcare; and
WHEREAS, the parties agree that increase in weekly hours of service in the areas of dental
care and psychiatry is needed pursuant to Article I (C) of the Agreement; and
NOW, THEREFORE, in accordance with the terms for modification of the Agreement and
for good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged,
Weld County and CCS agree as follows:
1. The fee to be paid on a monthly basis by Weld County to CCS is hereby increased
to reflect the change in the Consumer Price Index ("CPI") for medical services in
the Denver -Boulder -Greeley metropolitan area from June 2012 through June 2013.
The CPI increase for the aforementioned time period is 3.9%, thereby increasing
the compensation to be paid to CCS by $118,521, calculated on an annual basis.
2. The addition of 2 (two) hours weekly for the professional services of a dentist and
dental assistant and 4 (four) hours weekly for the professional services of a
psychiatrist hereby increases the compensation to be paid by Weld County
to CCS by $15,478 and $42,110, respectively, for a cumulative increase of $57,588
annually for staffing.
3. The total increase in costs for all changes to the Agreement as addressed by this
Amendment will be $176,109. Therefore the revised annual compensation will be
$3,215,109 which will be paid in monthly installments of $267,925.75 (Two
Hundred Sixty -Seven Nine Hundred Twenty -Five Dollars and Seventy -Five Cents)
for the twelve months of Year 2014.
4. The staffing plan ("Exhibit B") appended hereto and fully incorporated into this
Amendment by reference replaces the original Exhibit B to the Agreement.
5. All terms and conditions of the Agreement not expressly amended herein will
remain in full force and effect.
IN WITNESS WHEREOF, Weld County and CCS have executed this Amendment on
the most recent date indicated below.
