RE: APPROVE MEMORANDUM OF UNDERSTANDING AND AUTHORIZE CHAIR TO
SIGN - COLORADO ASSOCIATION OF AREA AGENCIES ON AGING
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Memorandum of Understanding
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, Area Agency
on Aging, and the Colorado Association of Area Agencies on Aging, regarding the Colorado
Health Foundation grant funds, commencing July 1, 2013, and ending June 30, 2015, with further
terms and conditions being as stated in said memorandum of understanding, and
WHEREAS, after review, the Board deems it advisable to approve said memorandum of
understanding, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Memorandum of Understanding between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, by and through
the Board of County Commissioners of Weld County, on behalf of the Department of Human
Services, and the Colorado Association of Area Agencies on Aging, regarding the Colorado
Health Foundation grant funds, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said memorandum of understanding.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 23rd day of September, A.D., 2013, nunc pro tunc July 1, 2013.

Regarding the Colorado Health Foundation Grant Funds
Enclosed for Board approval is a Memorandum of Understanding between the Department's
Area Agency on Aging and the Colorado Association of Area Agencies on Aging regarding
the Colorado Health Foundation Grant funds. This Memorandum of Understanding was
reviewed under the Board's Pass -Around Memorandum dated July 17, 2013, and approved
for placement on the Board's Agenda.
The Colorado Health Foundation (CHF) grant award to Colorado Association of Area Agencies on
Aging (C4A) is $1,016,400 over 24 months, to provide streamlined access to Long -Term Services
and Supports (LTSS). Of that total granted amount, $44,000 in year 1 will be provided to Weld
County Department of Human Services Area Agency on Aging and $50,000 will be provided in
Year 2. The amount awarded in Year 2 is subject to change, based on funding opportunities that
may be provided through the State Unit on Aging (SUA) and at the discretion of the CHF.
The Weld County Area Agency on Aging will be reporting units of services as indicated in the
MOU. We will work with the Colorado Health Foundation regarding ARCH development across
the state and will be discussing issues such as sustainability and scope of service. There is no match
requirement of the grant.
The term of this MOU is from July 1, 2013 through June 30, 2015.

Memorandum of Understanding

by and through the Weld County Board of Commissioners
Regarding the Colorado Health Foundation grant funds
Parties to this agreement
The parties to this memorandum are the Colorado Association of Area Agencies on Aging (C4A)
and the Weld County Department of Human Services Area Agency on Aging,
by and through the Weld County Board of Commissioners (WCDHS AAA).
II. Purpose
The purpose of this Memorandum of Understanding (MOU) is to set forth the roles and responsibilities of
each party in the use of funds granted by the Colorado Health Foundation (CHF) to develop and strengthen
the Adult Resources for Care and Help (ARCH) network, the Aging and Disability Resource Center (ADRC)
program for Colorado.
III. Terms
The term of this MOU is from July 1, 2013 through June 30, 2015.
IV. Summary of the Adult Resources for Care and Help (ARCH) program
The Adult Resources for Care and Help (ARCH) program, which is the Colorado name for the Aging
and Disability Resource Center (ADRC) program, provides streamlined access to Long -Term Services
and Supports (LTSS) to seniors (adults 60 and older) and adults living with disabilities, through both
Information and Referral services and Options Counseling. These services and supports are
provided with the goal of helping consumers to live as independently as possible, while remaining
in their communities. Information and Referral services are provided state-wide through all of the
Area Agencies on Aging (AAAs), as well as the Colorado 2-1-1 network. Options Counseling, a
consumer -directed form of short-term case management, is provided through the ARCH program
sites to those consumers who are unable to access the resources available in their region without
assistance.
The goal of the ARCH program is to use Information and Referral services and Options Counseling
to enable the service providers and the consumers to work together to best fit the services
required to meet the needs of the consumer. Using a consumer -directed multipronged approach,
consumers who are able to navigate long-term care systems on their own are given the information
they need, while those who require more involvement by an options counselor, may be helped
every step of the way.
V. Summary of Colorado Health Foundation grant award
The Colorado Health Foundation (CHF) grant award to C4A is $1,016,400 over 24 months, to provide

be provided to Weld County Department of Human Services Area Agency on Aging in Year 1 and $50,000 will

State Unit on Aging (SUA) and at the discretion of the CHF.
VI. Statement of Work and Responsibilities
Beginning July 1, 2013 through June 30, 2015, duties for Weld County Department of Human Services Area
Agency on Aging under the terms of this MOU will include, but will not be limited to:

• Participate in and support the commitment of the ARCH network state-wide to report to the State
Unit on Aging the individuals from whom information/referral contacts were received and
unduplicated options counseling service units were provided, as shown in the table below which
relates to your particular AAA Region:

• Participate in and support the commitment of the ARCH network state-wide to report to the State

• Collaborate with the contracted consultant to develop, in coordination with C4A and TCHF, and
begin implementation of an evaluation plan of ARCH, which may include factors of cost-
effectiveness and/or sustainability
• Continue to Incorporate and strengthen strategies to better integrate the disability community with
ARCH practices.
o Each ARCH site will review resources with the AAA, ILC and CCB in their area to confirm the
accuracy of referrals.
o Each ARCH site will partner with its local AAA, ILC and CCB to create and execute an outreach
strategy to inform all consumers about ARCH services.
o Each ARCH will work with its local AAA, ILC and CCB to prevent duplication of options counseling
services in the region.
In Year 2:
• Please refer to table in Year 1 Intermediate Milestones for information/referral and options counseling
service goals for Year 2.
• Please refer to table in Year 1 Intermediate Milestones for enrollment of individuals in Medicaid LTSS
goals for Year 2.
• Collaborate in the final evaluation and report, which will be included in the final grant report to the
Colorado Health Foundation.
• Collaborate in the finalization of an ARCH site business plan that includes a model for sustainability
By signing this MOU, both parties agree to abide by the terms listed herein for ARCH services provided under the
Colorado Health Foundation funding through the Colorado Association of Area Agencies on Aging.
