RE: APPROVE AMENDMENT TO 2012-2013 AGREEMENT FOR COLORADO
PREVENTION PARTNERSHIP FOR SUCCESS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an amendment to the 2012-2013
Agreement for the Colorado Prevention Partnership for Success between the County of Weld,
State of Colorado, by and through the Board of County Commissioners of Weld County, on
behalf of the Department of Human Services, to the Colorado Department of Human Services,
with terms and conditions being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the amendment to the 2012-2013 Agreement for the Colorado
Prevention Partnership for Success between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Department of
Human Services, to the Colorado Department of Human Services, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said application.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 20th day of February, A.D., 2013.

RE: Amendment to the 2012-2013 Agreement for the Colorado
Partnership for Success Program between the Weld County
Department of Human Services and the Colorado Department
of Human Services, Office of Behavioral Health and with
North Range Behavioral Health
Enclosed for Board approval is an Amendment to 2012-2013 Agreement for the Colorado
Partnership for Success Program between the Weld County Department of Human Services and
the Colorado Department of Human Services, Office of Behavioral Health and with North Range
Behavioral Health. This Amendment was reviewed under the Board's Pass -Around
Memorandum dated February 8, 2013, and approved for placement on the Board's Agenda.
This amendment has no total impact on the $400,000 budget; however, it moves $17,000 from
the "Personnel Costs" category to the "Consultants" and "Operating" categories, $8,500 each.
This amendment was made possible because North Range Behavioral Health combined two part-
time positions into one full-time position.
The Department acts as the fiscal agent for the Grant and North Range Behavioral Health
directly operates the Program.

Requested change calculation
Move $17,000.00 from personnel budget category to consultants and operating budget
categories.
Justification for requested change
Recent "new hire" 1-22-13" combining two part time evaluator positions into one has allowed
for a cost savings in the personnel budget categoy of a projection of $17,000.00. Moving
$8,500.00 each to the consultant budget categoy and the operating budget categories allows
for repurposing of the resources to staffing capacity/marketing and community/coalition
support.
