RE: APPROVAL OF WELD COUNTY DEPARTMENT OF SOCIAL SERVICES 1979
DENTAL CONTRACT BETWEEN WELD COUNTY, COLORADO AND THE WELD
COUNTY DENTAL SOCIETY.
WHEREAS, the Board of County Commissioners of Weld County,
Colorado, pursuant to Colorado statute and the Weld County Home
Rule Charter, is vested with the authority of administering the
affairs of Weld County, Colorado, and
WHEREAS, the Weld County Department of Social Services 1979
Dental Contract between Weld County, Colorado and the Weld County
Dental Society has been presented to the Board of County Commis-
sioners. A copy of said contract is attached hereto and incorporated
herein by reference, and
WHEREAS , the Board of County Commissioners deems it advisable
and in the best interests of Weld County to approve said contract.
NOW, THEREFORE, BF IT RESOLVED by the Board of County Commis-
sioners of Weld County, Colorado that the Weld County Department
of Social Services 1979 Dental Contract between Weld County, Colorado
and the Weld County Dental Society be, and hereby is, approved.
The above and foregoing Resolution was, on motion duly made
and seconded, adopted by the following vote on the 3rd day of
January, A.D. , 1979.

WELD COUNTY DEPARTMENT OF SOCIAL SERVICES
1979 DENTAL CONTRACT
This contract executed at Greeley, Colorado this 27 day of December, 1978, by
and between the Board of County Commissioners of Weld County, Colorado, herein-
after called the County, and the Weld County Dental Society, hereinafter called
the Society:
WITNESSETH:
That in consideration of the mutual promises and obligations and other considera-
tions herein expressed, it is mutually agreed until December 31, 1979:
1. The Weld County Dental Society shall furnish dental services as outlined in
the fee schedule for county indigents. Approved billings shall be submitted
bi-monthly. Payment is to be made to the Weld County Dental Society. The
budgeted annual cost for this service is $12,000.00. If this amount should
become depleted, no provision is made for continued services.
2. The Society will appoint an administrative committee to assist in checking
billings for services rendered.
3. The Society will appoint its own committee, empowered to make all arrangements
to properly administer and fairly divide the funds among participating members
rendering service, and conduct the business of the Society under this contract.
The decision of this committee shall be final and binding to all doctors who
participate in this contract. It is expressly understood, however, that any
member of the Weld County Dental Society may be present at any meeting of such
committee and have full opportunity to examine the books to see that the funds
are divided fairly and without prejudice among all participating doctors who
have performed services. The members of this committee act only as agent for
the Society and in no case shall they be held liable.
4. Any participating doctor suspected of irregular billings or falsifications of
his records may be disqualified from participating in the contract by the
Administrative Committee.
5. The Society will, so far as it lies within the power of its members, see that
proper dental care and service are rendered to all indigents in Weld County.
6. Maximum adult coverage shall be $90.00 per year. Weld County indigents who
receive dental services under this contract shall be liable for 10% of the
authorized service amount, to a maximum of $9.00.
THIS BENEFIT EXCLUDES DENTURES, DENTURE REPAIRS, OR OTHER EXTENSIVE SPECIALIZED
SERVICES.
7. It is understood by all parties to this contract that TITLE XIX of the Social
Security Act (MEDICAID) provides dental services for all children of eligible
public assistance recipients up to the age of 21. Billings for any services
rendered children covered by this MEDICAID program shall be made directly to
the fiscal agent by the provider.
8. Any unexpended funds at the end of the year 1979 shall revert to Weld County,
Colorado.

IN WITNESS THEREOF, said parties have subscribed and executed these presents,
the County in its proper name and by its proper authorized officers, attested
by its Clerk and Seal, and said Society by its proper name and proper officers
thereunto duly authorized by it, the date first shown mentioned.

Please find attached the 1979,4ental contract between the Board of Lounty
Commissioners and the Weld County Dental Society. The contract is very
similar to previous contracts that we have had over the years and is funded
from the budgeted monies in the General Assistance fund.
